Welcome to the Universal Housing Rental Assistance Online Application Collection System
Information needed: Names, Social Security numbers, birth dates and income information for everyone in the household.
Eligibility: Once your application status is "Active". You must provide the following documents for all members of your Household as verification within 48 hours:
- Photo ID
- Birth Certificate
- Proof of Income
- Social Security Card
**Log into www.assistancecheck.com with the same account you use to submit your application online through this WaitListCheck System to upload a copy of the requested documents electronically.**
How to Apply: You must have an email address to complete this Preliminary Application. This Preliminary Application must be submitted electronically using this system to create a receipt of application. Paper copies of this form may be obtained by calling your local Housing Agency.
