to the PHA’s collection and use of family information as provided for in

- Not
currently be receiving a duplicative subsidy (voucher, rental
assistance)

COMPLETE THE APPLICATION. ALL INFORMATION MUST BE ACCURATE, CURRENT
& COMPLETED IN FULL TO PROCESS.
- Full
Name(s) of all current members in the household including yourself
- Social
Security Numbers of all members in the household including yourself
- Birth
Dates of all members in the household including yourself
- Proof
of Current Address Uploaded to Assistancecheck.com (ID, or Utility Bill
with Current Address)
- ALL sources of Income for
all members living in household must be reported accurately at time of
application. Sources of Income Include:
Employment Wages, SSI, SSD, SNAP, Unemployment, Child Support, Assets,
Retirement Benefits, Alimony, Assistance from other Family Members,
Workers Compensation, Veterans Benefits, Settlements, Inheritance, Rental
Income etc.

APPLICATION REVIEW AND REMINDERS:
- TO
CHECK YOUR STATUS ON THE WAITLIST, PLEASE WAIT 3 DAYS AND CREATE
YOUR ACCOUNT FOR ASSISTANCE CONNECT. PLEASE
UPLOAD AND SUBMIT A COPY OF YOUR PROOF OF ADDRESS ON ASSISTANCE CONNECT AT THIS
TIME IN ORDER FOR YOUR APPLICATION TO FULLY PROCESS AND BE ACCEPTED.
- IF
YOU MOVE TO A NEW ADDRESS, YOU MUST REPORT AND UPDATE THE ADDRESS ON ASSISTANCE
CONNECT OR YOU WILL NOT BE NOTIFIED WHEN VOUCHER ASSISTANCE BECOMES AVAILABLE. IF
YOU HAVE NOT PROVIDED AN UPDATED ADDRESS TO THE OFFICE, YOU WILL NOT RECEIVE
INFORMATION WHEN PULLED AND WILL HAVE TO REAPPLY TO THE PROGRAM’S WAITING LIST.
- PLEASE BE PATIENT. IT MAY TAKE YEARS TO BE
PULLED FROM THE WAITLIST. YOU CAN CHECK YOUR STATUS AND COMMUNICATE WITH THE
OFFICE ON ASSISTANCE CONNECT.
- ONCE SELECTED FROM THE WAITLIST, YOU WILL RECEIVE A TENANT INFORMATION PACKET AT THE ADDRESS YOU HAVE PROVIDED ON FILE. YOU WILL HAVE 10 BUSINESS DAYS FROM POST MARK DATE ON MAIL TO COMPLETE AND RETURN THIS PACKET TO THE OFFICE IN ORDER TO BE ELIGIBLE FOR SUBSIDY ASSISTANCE.
